Free Sports Physicals for Next Year!

Save the Date - May 18th

Attention Parents/Guardians and Students entering 6th–12th grade for the 2026-2027 school year:

On Monday, May 18th, TWISD is partnering with Family N Friends Orthopedics (FNF) to provide free sports physicals for all students! This event will take place in the elementary admin office from 5:30-7:30 pm on a first-come first-serve basis.

  • It is preferred that a parent come with the student
  • Each student must have an updated medical history form completely filled out and signed by a parent prior to completing the physical
If you cannot or choose not to attend this event, your child can complete their physical with their primary doctor or other provider of your choice. Some clinics and some chiropractors may also provide sports physicals. Costs may vary - contact your provider for more details.
If your student plans to participate in UIL athletics next school year, UIL requires several forms including:

📝 1. Medical History Form
  • This form must be completed and signed by both the student and a parent/guardian every year, regardless of whether a new physical is required.
  • This helps us stay up to date on any medical conditions we need to be aware of.
🩺 2. Pre-Participation Physical Exam
  • A physical exam must be completed by a healthcare provider.
  • It must be dated after May 1, 2026 to be valid for the 2026-2027 school year.
  • Physicals are required for all incoming 6th, 7th, 9th, and 11th graders, and for any student who does not already have a current physical on file.

This is applicable to ALL UIL sports for ALL students in grades 6-12.

Both forms are available through Nurse Allison, the Athletics Department, on the UIL website, are attached to this post, and will be available at the May 18th health fair. All forms must be submitted to Nurse Allison before the first day of practice (including summer practice).
