If your student plans to participate in UIL athletics next school year, UIL requires several forms including:
📝 1. Medical History Form
- This form must be completed and signed by both the student and a parent/guardian every year, regardless of whether a new physical is required.
- This helps us stay up to date on any medical conditions we need to be aware of.
🩺 2. Pre-Participation Physical Exam
- A physical exam must be completed by a healthcare provider.
- It must be dated after May 1, 2026 to be valid for the 2026-2027 school year.

Physicals are required for all incoming 6th, 7th, 9th, and 11th graders, and for any student who does not already have a current physical on file.
This is applicable to ALL UIL sports for ALL students in grades 6-12.
Both forms are available through Nurse Allison, the Athletics Department, on the
UIL website, are attached to this post, and will be available at the May 18th health fair.
All forms must be submitted to Nurse Allison
before the first day of practice (including summer practice).
